Medication in the academy

Where a child needs prescription medication in the Academy, we must have a completed form with all the details, giving us permission to administer the medication. The most common forms of prescription medication required in the Academy are items like inhalers, epipens and eczema cream.

Antibiotics that are required three times a day, should be taken at home. We will only accept antibiotics in the Academy that have to be administered four times a day. These will need to be signed in and signed out.

What you need to do:

  • Download and fill in the form below or collect a paper copy from the Academy office
  • Bring the medication and the form into the Academy in it's original box with the prescription label on
  • We will then check it against the form, have it signed off by a senior member of staff and store it for your child's use.

If your child does not need their inhaler or other prescription in the Academy, you will need to complete this medical disclaimer form.

Non Prescription Medication:

For any non-prescription items such as hand cream, you will need to complete the non prescription medication form.

This includes items such as:

  • hand cream
  • pain relief
  • lip salve
  • suncream
