Medication in the academy

Where a child needs prescription medication in the Academy, we must have a completed form with all the details, giving us permission to administer the medication. The most common forms of prescription medication required in the Academy are items like inhalers, Epi-pens and eczema cream.

Antibiotics that are required three times a day, should be taken at home. We will only accept antibiotics in the Academy that have to be administered four times a day. These will need to be signed in and signed out.

What you need to do:

  • Download and fill in the form below or collect a paper copy from the Academy office
  • Bring the medication and the form into the Academy in it's original box with the prescription label on
  • We will then check it against the form, have it signed off by a senior member of staff and store it for your child's use.
